Please complete this CE credit request if you completed or registered for PFSA’s Recognizing and Reporting Child Abuse training and have a license under the PA Department of State which requires proof of mandated reporter training to renew (or are applying for such a license). Enter your information exactly as it appears on your health-related license or license application. If you do not yet have a license, enter “new” for the license number. 

After we verify your completion of the training, we will submit this information to the Pennsylvania Department of State for your license renewal/application. Please allow 7 to 10 days for the processing of your credits.  You may monitor the status of your license on-line at http://www.pals.pa.gov/  If after 10 days your credits have not been applied, contact us at training@pafsa.org or 717-238-0937.
